Educuras is partnered with Kaiser Permanente to provide Enhanced Care Management (ECM).

A free program that helps eligible Medi-Cal members improve their health, stay connected to care, and access essential community support. Whether you’ve recently been in the hospital, have complex health needs, or are facing challenges like housing or transportation, our care team is here to help.

Who We Serve

Now accepting Medi-Cal patients in the following counties:

• Los Angeles

• San Diego

• Riverside

• San Bernardino

• Orange

• Ventura

• Kern

• Imperial

What We Offer

A dedicated care manager and personalized care plan

Support after emergency room or hospital discharge

Referrals for housing, food, transportation, and social services

Coordination for medical appointments and medication support

Health education for managing chronic conditions like diabetes, high blood pressure, and mental health

Regular check-ins and ongoing support from a trusted care team

Who Qualifies

Educuras ECM supports all Medi-Cal Populations of Focus (POF), including:

  • Individuals experiencing homelessness

  • Individuals at risk for frequent ER or hospital visits

  • Individuals with serious mental health or substance use needs

  • Individuals transitioning from incarceration

  • Adults at risk for long-term care or living in nursing facilities transitioning to the community

  • Children and youth in California Children’s Services (CCS) or Whole Child Model (WCM)

  • Children and youth involved in child welfare

  • Individuals with intellectual or developmental disabilities (I/DD)

  • Pregnant and postpartum individuals (birth equity focus)
