Who may qualify
ECM is designed for Medi-Cal members with complex health and social needs. If any of the below describes you or a loved one, you may qualify.

Frequently asked questions
Do I have to pay for Enhanced Care Management?
No. Enhanced Care Management through Educuras is free for eligible Kaiser Permanente Medi-Cal members.
How often will I hear from my care manager?
You can reach out to your care manager any time you need support. At a minimum, you will connect at least twice a month. Support comes through a mix of in-person visits and virtual check-ins to fit your schedule.
Can Educuras help with housing or food?
Yes. We coordinate referrals and connect members to community resources that support health, including housing, food, and other essential services.
What languages do you offer?
Our care team delivers support in English and Spanish — in the language you prefer, not translated as an afterthought.
Do I need a doctor's referral to enroll?
No. You can refer yourself directly. We will check eligibility with Kaiser Permanente once we receive your referral form.
