Enhanced Care Management (ECM)

Extra support for your health — when you need it most.

Our Enhanced Care Management (ECM) program is free for Kaiser Permanente Medi‑Cal members.

You’ll meet with your care manager in person each month, with ongoing virtual check‑ins and support in between. This is hands‑on care coordination — personalized support, real connection, and a dedicated team to help you navigate your health and community resources with confidence.


Who We Serve

Now accepting Medi-Cal patients in the following counties:

  • Los Angeles

  • San Diego

  • Riverside

  • San Bernardino

  • Orange county

  • Ventura

  • Kern

  • Imperial


What We Offer

✓ A dedicated care manager and personalized care plan

✓ Check-in after emergency room or hospital discharge

✓ Support with referrals for housing, food, transportation, and social services

✓ Coordination for medical appointments and medication support

✓ Health education for managing chronic conditions like diabetes, high blood pressure, and mental health

✓ Regular virtual/in-person check-ins and ongoing support from a trusted care team

Who Qualifies

Educuras ECM supports the following Medi-Cal Populations of Focus (POF), including:

  • Individuals experiencing homelessness

  • Individuals at risk for frequent ER or hospital visits

  • Individuals with serious mental health or substance use needs

  • Adults at risk for long-term care or living in nursing facilities transitioning to the community

  • Children and youth in California Children’s Services (CCS) or Whole Child Model (WCM)

  • Children and youth involved in child welfare

  • Individuals with intellectual or developmental disabilities (I/DD)

  • Pregnant and postpartum individuals (birth equity focus)
