Who qualifies
The program is designed for individuals with prediabetes or those at high risk. You may qualify if any of the following describes you.
High blood sugar
A1C in the prediabetes range (5.7%–6.4%)
Gestational diabetes
A history of diabetes during pregnancy
High BMI
Being overweight based on clinical guidelines
CDC risk score
A high score on the official CDC prediabetes risk test
